26-27 HCCS Limited Criminal History Form

Filling out the Limited Criminal History Form will fulfill all background checks for all HCCS schools. Only one background check needed.

The Cash Tuition (Out of District) Request Form is to be completed by parents/guardians who wish to have their child attend Huntington County Community Schools but the student does not have a parent/legal guardian residing within Huntington County.  A tuition transfer request form must be submitted for EACH eligible transfer student.  HCCS currently does not charge any fees for Indiana students to attend HCCS.

Facility Use Agreement

A request to transfer a student within Huntington County Community Schools must be made on this form. A separate form must be completed for each student. The School Board Policy for this request is printed on page two of this form.

This form only pertains to those who have not enrolled in our district for the previous school year.

School Secretaries will contact you to complete registration.

To prove residency for school in Indiana, parents or legal guardians need to provide documents showing their name and current address.

Proof of Residency Form

Due to state regulations, we are required to collect Proof of Residency for all students (new and returning).
